CLINICAL PHARMACOLOGY Nafarelin acetate is a potent agonistic analog of gonadotropin-releasing hormone GnRH ; . At the onset of administration, nafarelin stimulates the release of the pituitary gonadotropins, LH and FSH, resulting in a temporary increase of ovarian steroidogenesis. Repeated dosing abolishes the stimulatory effect on the pituitary gland. Twice daily administration leads to decreased secretion of gonadal steroids by about 4 weeks; consequently, tissues and functions that depend on gonadal steroids for their maintenance become quiescent. Nafarelin acetate is rapidly absorbed into the systemic circulation after intranasal administration. Maximum serum concentrations measured by RIA ; were achieved between 10 and 40 minutes. Following a single dose of 200 g base, the observed average peak concentration was 0.6 ng mL range 0.2 to 1.4 ng mL ; , whereas following a single dose of 400 g base, the observed average peak concentration was 1.8 ng mL range 0.5 to 5.3 ng mL ; . Bioavailability from a 400 g dose averaged 2.8% range 1.2 to 5.6% ; . The average serum half-life of nafarelin following intranasal administration is approximately 3 hours. About 80% of nafarelin acetate is bound to plasma proteins at 4C. Twice daily intranasal administration of 200 or 400 g of SYNAREL in 18 healthy women for 22 days did not lead to significant accumulation of the drug. Based on the mean Cmin levels on Days 15 and 22, there appeared to be dose proportionality across the two dose levels. After subcutaneous administration of 14C-nafarelin acetate to men, 4455% of the dose was recovered in urine and 18.544.2% was recovered in feces. Approximately 3% of the administered dose appeared as unchanged nafarelin in urine. The 14C serum half-life of the metabolites was about 85.5 hours. Six metabolites of nafarelin have been identified of which the major metabolite is Tyr-D 2 ; -Nal-Leu-Arg-Pro-GIy-NH2 5-10 ; . The activity of the metabolites, the metabolism of nafarelin by nasal mucosa, and the pharmacokinetics of the drug in hepatically- and renally-impaired patients have not been determined. There appeared to be no significant effect of rhinitis, i.e., nasal congestion, on the systemic bioavailability of SYNAREL; however, if the use of a nasal decongestant for rhinitis is necessary during treatment with SYNAREL, the decongestant should not be used until at least 2 hours following dosing of SYNAREL. In controlled clinical studies, SYNAREL at doses of 400 and 800 g day for 6 months was shown to be comparable to danazol, 800 mg day, in relieving the clinical symptoms of endometriosis pelvic pain, dysmenorrhea, and dyspareunia ; and in reducing the size of endometrial implants as determined by laparoscopy. The clinical significance of a decrease in endometriotic lesions is not known at this time and, in addition, laparoscopic staging of endometriosis does not necessarily correlate with severity of symptoms. In a single controlled clinical trial, intranasal SYNAREL nafarelin acetate ; at a dose of 400 g per day was shown to be clinically comparable to intramuscular leuprolide depot, 3.75 mg monthly, for the treatment of the symptoms dysmenorrhea, dyspareunia and pelvic pain ; associated with endometriosis.

Where specific symptoms predominate, symptom specific therapies may be appropriate. For severe anxiety or sleep disturbance alprozolam 025 mg b.i.d. or triazelam 025 mg qhs respectively may be helpful. Estrogen withdrawal has been linked to menstrually-related migraines and estrogen supplementation with a 100 ug transdermal estrogen patch or the acute administration of 1 mg estradiol sublingually at the time of migraine aura may be helpful to avert menstrual migraine. In this group of patients therefore, it is essential to start any anti-psychotic medication at very low doses, so as to avoid the unpleasant side-effects that typically occur with these drugs and which contribute to poor compliance in the long-term. The most problematic side effects are the extra-pyramidal side effects EPSE ; . There are three types of EPSE, which may occur in the initial treatment period. Then the PSL tablet formulation was altered from Tablet A to Tablet B with the same treatment regimen, but the platelet counts fell drastically thereafter. However, the platelet counts recovered by changing the PSL tablet formulation back from Tablet B to Tablet A. In vitro dissolution testing was undertaken to assess bioequivalence between Tablet A and Tablet B. PSL in Tablet B was released more slowly compared with that in Tablet A regardless of the medium pH conditions, and the diSerence in the release rate between the two tablet formulations increased with increasing medium pH value. The diSerence exceeded the allowance limit 15 ; for judgement of bioequivalence under conditions above pH 4, indicating that Tablet A and Tablet B might be nonbioequivalent. The intragastric pH of the patient was probably raised due to coadministration of cimetidine. Therefore the present results suggest that the disparity in the immunosuppressive eSects between the two PSL tablet formulations was attributable to the diSerence in their dissolution behavior in the gastrointestinal tract. We consider that it is better to avoid interchanging PSL tablet formulations in clinical practice. Boulton University Dept of Medicine and Diabetes, Manchester Royal Infirmary, Manchester M13 0JE Background objective Methicillin-resistant Staphylococcus aureus MRSA ; is an increasing problem amongst diabetic patients with foot ulceration and is reported to be associated with slower healing rates. We hypothesized that larval therapy would eradicate MRSA colonization in diabetic foot ulcers and we report the results of our preliminary observational study. Methods Thirteen diabetic patients 10 males ; aged 64 years range 4679 ; with 15 MRSA positive foot ulcers of 11.6 weeks duration were included. All patients were consecutively seen by the diabetes team and had no clinical and or laboratory signs of systemic infection. Sterile free-range larvae were applied for 4 days, at densities determined by the larvaE calculator. We used a minimum of two applications and a maximum of seven for each ulcer. MRSA status was evaluated by semi-quantitative wound tissue cultures taken prior to each larvae application. A topical MRSA eradication protocol was followed in each patient with positive MRSA body screening cultures.
