How are we to understand the push for neo-liberal reform in contexts where public goods are provided relatively well? From a neo-liberal perspective, state-led development represents a failed paradigm. State-owned enterprises do not possess the efficiency, resources, or autonomy required to promote growth, macroeconomic stability, or development. These explanations have at their root an important insight regarding the dangers of political interference that can lead to rent-seeking and productive inefficiencies Krueger, 1974 ; . This deep concern with state failure is coupled with the assumption that market failures are less costly and intransigent. Establishing a liberal economic order is thus proposed and implemented as a solution to the inadequacies of state-led development. In the words of Margaret Thatcher, "TINA: there is no alternative." If true, the main concern becomes identifying and overcoming impediments to reform. Scholars from the "politics of economic adjustment" perspective deepened this debate by addressing the question of how collective action problems arise and are overcome Nelson, 1990; Haggard and Kaufman, 1992 ; . They argued that because potential winners are diffuse and only benefit at indeterminate points in the future, while potential losers are specific, organized actors whose interests will be quickly and clearly harmed, neo-liberal reform would be limited where autonomy from interest groups was not secured. The success of reform projects was thus dependent upon a strong, autonomous team of technocrats that could direct the economy without political interference from interest groups and videx.
Drug Resistance: HIV isolates with reduced sensitivity to zidovudine have been selected in vitro and were also recovered from patients treated with RETROVIR. Genetic analysis of the isolates showed mutations that result in 5 amino acid substitutions Met41Leu, A67Asn, Lys70Arg, Thr215Tyr or Phe, and Lys219Gln ; in the viral reverse transcriptase. In general, higher levels of resistance were associated with greater number of mutations with 215 mutation being the most significant. Cross-Resistance: The potential for cross-resistance between HIV reverse transcriptase inhibitors and protease inhibitors is low because of the different enzyme targets involved. Combination therapy with zidovudine plus zalcitabine or didanosine does not appear to prevent the emergence of zidovudine-resistant isolates. Combination therapy with RETROVIR plus EPIVIR delayed the emergence of mutations conferring resistance to zidovudine. In some patients harboring zidovudine-resistant virus, combination therapy with RETROVIR plus EPIVIR restored phenotypic sensitivity to zidovudine by 12 weeks of treatment. HIV isolates with multidrug resistance to zidovudine, didanosine, zalcitabine, stavudine, and lamivudine were recovered from a small number of patients treated for 1 year with the combination of zidovudine and didanosine or zalcitabine. The pattern of resistant mutations in the combination therapy was different Ala62Val, Val75Ile, Phe77116Tyr, and Gln151Met ; from monotherapy, with mutation 151 being most significant for multidrug resistance. Site-directed mutagenesis studies showed that these mutations could also result in resistance to zalcitabine, lamivudine, and stavudine. CLINICAL PHARMACOLOGY Pharmacokinetics: Adults: The pharmacokinetic properties of zidovudine in fasting patients are summarized in Table 1. Following oral administration, zidovudine is rapidly absorbed and extensively distributed, with peak serum concentrations occurring within 0.5 to 1.5 hours. Binding to plasma protein is low. Zidovudine is primarily eliminated by hepatic metabolism. The major metabolite of zidovudine is GZDV ; . GZDV area under the curve AUC ; is about 3-fold greater than the zidovudine AUC. Urinary recovery of zidovudine and GZDV accounts for 14% and 74%, respectively, of the dose following oral administration. A second metabolite, 3-amino-3-deoxythymidine AMT ; , has been identified in the plasma following single-dose intravenous IV ; administration of zidovudine. The AMT AUC was one fifth of the zidovudine AUC. Pharmacokinetics of zidovudine were dose independent at oral dosing regimens ranging from 2 mg kg every 8 hours to 10 mg kg every 4 hours. The extent of absorption AUC ; was equivalent when zidovudine was administered as RETROVIR Tablets or Syrup compared to RETROVIR Capsules.

There are currently seven approved nucleoside analogue reverse transcriptase inhibitors. Data are available from clinical trials in human pregnancy for zidovudine, lamivudine, didanosine, and stavudine. Abacavir, emtricitabine, and zalcitabine have not been studied in pregnant women. Tenofovir disoproxil fumarate is the first nucleotide analogue reverse transcriptase inhibitor. The nucleoside analogue drugs require three intracellular phosphorylation steps to form the triphosphate nucleoside, which is the active drug moiety; tenofovir, an acyclic nucleotide analogue drug, contains a monophosphate component attached to the adenine base, and hence only requires two phosphorylation steps to form the active moiety. However, in the rabbit, no evidence of drug-related developmental toxicity was observed and no increase in fetal malformations was observed at doses up to 700 mg kg about 8.5 times that of human therapeutic exposure ; . Placental and breast milk passage Abacavir crosses the placenta and is excreted into the breast milk of lactating rats. Human studies in pregnancy No studies have been conducted with abacavir in pregnant women or neonates. Serious hypersensitivity reactions have been associated with abacavir therapy in non-pregnant adults and have rarely been fatal; symptoms include fever, skin rash, fatigue, and gastrointestinal symptoms such as nausea, vomiting, diarrhea, or abdominal pain. Abacavir should not be restarted following a hypersensitivity reaction because more severe symptoms will recur within hours and may include life-threatening hypotension and death and dipyridamole.
The vast majority of Iraq's 13 million children will be affected if the war between Iraq and the West goes ahead, a report released by a Canadian led team of health experts concludes. The report, based on a humanitarian fact finding mission in Iraq that included interviews with 100 families in Baghdad, Karbala, and Basra in January 2003, warns that Iraqi children are at grave risk of starvation, disease, death, and psychological trauma. Casualties are predicted to be in the tens of thousands in the event of war. Referring to the decline in health and nutritional status during 12 years of economic sanctions after the Gulf war, Dr Samantha Nutt, the team's public health expert, said, "Iraqi children are more vulnerable than ever." Two child psychologists who accompanied the team interviewed more than 300 children to assess their mental health. They found that the children have "a great fear" of a war they perceive to be "hanging over their heads." Children as young as four described clear ideas about the horrors of war. They are fearful, anxious, and depressed about the prospect of armed conflict, the report says. Many have nightmares, and 40% do not think that life is worth living. 5.1 Exposure data Hydroxyurea is a chemically simple antimetabolite that inhibits the enzyme ribonucleotide reductase. It has been in clinical use since the 1960s and is widely used for the treatment of severe sickle-cell disease, chronic myeloid leukaemia, myeloproliferative disorders such as polycythaemia vera and essential thrombocythaemia and, increasingly, in combination with didnaosine in HIV infection. Hydroxyurea is sometimes used for the treatment of psoriasis and various solid tumours. 5.2 Human carcinogenicity data The risk for leukaemia associated with administration of hydroxyurea in the treatment of chronic myeloproliferative disorders has been evaluated in a number of small cohort studies. Overall, 56% of patients developed either acute leukaemia or myelodysplastic syndrome subsequent to the start of hydroxyurea treatment. Large variation in the length of active follow-up was not taken into account in the analyses. The risk for leukaemia in patients with chronic myeloproliferative disorders who were not treated with hydroxyurea or other agents e.g. polycythaemia vera patients treated with phlebotomy alone ; was also increased in comparison with that of the general population. The available data do not allow a conclusion about whether the occurrence of acute leukaemia and myelodysplastic syndrome in the hydroxyurea-treated patients represents progression of the myeloproliferative process or an effect of the treatment. 5.3 Animal carcinogenicity data Hydroxyurea was tested in one experiment in mice by intraperitoneal administration beginning at two days of age. No increase in the incidence of tumours was reported. Hydroxyurea has also been tested in combination with other chemical carcinogens to assess the effect of inhibition of DNA synthesis on carcinogenesis. The experiments are inadequate to assess the carcinogenicity of hydroxyurea. 5.4 Other relevant data Hydroxyurea is readily absorbed after oral administration. In one study, 35% of an administered dose was excreted unchanged in the urine of humans. Hydroxyurea is widely distributed in tissues. Its main toxic effect is neutropenia. Hydroxyurea is teratogenic and causes postnatal behavioural deficits after prenatal exposure in all species of animals in which it has been tested. It has commonly been used as positive control substance in studies of developmental toxicity. In one study of patients treated with hydroxyurea for essential thrombocythaemia who developed leukaemia, a statistically non-significant association was found with a 17p chromosomal deletion in leukaemic cells. Hydroxyurea neither bonds chemically nor otherwise binds to DNA. Instead, it inhibits ribonucleotide and vibramycin.

The nucleoside reverse transcriptase inhibitors nRTIs ; were the first antiretroviral drugs introduced into clinical practice and represent the cornerstone of highly active antiretroviral therapy HAART ; . These drugs are associated with various adverse effects Table 1 ; , including myelosuppression, pancreatitis, gastrointestinal GI ; intolerance, peripheral neuropathy, myopathy, and lactic acidosis. Many of these adverse effects are postulated to be mediated through mitochondrial toxicity.18, 19 Some studies have indicated that women are less likely than men to tolerate regimens containing certain nRTIs. In the ACTG 175 trial, 20 monotherapy with zidovudine or dodanosine was compared with combination therapy with zidovudine and zalcitabine or with zidovudine and didanosine. Reduction in drug dosage or discontinuation of didanosine-containing regimens because of adverse effects was more common among women than among men. Another study21 evaluated the incidence of adverse events during nRTI therapy in 1450 subjects with a CD4 + cell count of less than 500 L. The risk of developing an adverse event related to didanosine was nearly 3 times higher for women than for men relative risk, 2.7; P .03 ; . Lactic acidosis is one of the most serious presentations of nRTI-associated mitochondrial toxicity. Although this complication is rare, the associated mortality rate may be as high as 80% when plasma lactate levels exceed 10 mmol L.22 The role of sex in the occurrence of this complication has not been directly studied, but a report from the US Food and Drug Admin.
