The Food and Agriculture Organization of the United Nations FAO ; and the World Health Organization WHO ; have stated that there is adequate scientific evidence to indicate that there is potential for probiotic foods to provide health benefits and that specific strains are safe for human use 38 ; . An expert panel commissioned by FAO and WHO defined probiotics as "Live microorganisms which when administered in adequate amounts confer a health benefit on the host." This is the definition that should be used, and probiotics should not be referred to as biotherapeutic agents 84 ; . Probiotics represents an expanding research area. A Medline search of the term probiotics illustrates the significant increase in research undertaken in this area during the past 5 years: over 1, 000 publications cited, compared to 85 for the previous 25 years. While this demonstrates the potential significance of this emerging field, much still remains to be done to standardize the meaning of the term probiotic and which strains actually fulfill the criteria of true probiotic microorganisms. In addition, although clinical evidence of the tangible benefits of probiotics is mounting, this does not yet reflect the commercial front. Unfortunately, many so-called probiotic products have not been properly identified, documented, manufactured under good manufacturing practices, or proven clinically, yet various companies make claims that lead consumers and caregivers to believe that they are using reliable products. Thus, the establishment of standards and guidelines represents a necessary first step in making sure that probiotic products are indeed legitimate and effective. Such standards and guidelines have recently been generated and will be presented later. PRESENT STATUS OF PROBIOTICS IN CLINICAL PRACTICE It is important to first examine the present status of probiotics in clinical practice and the evidence of their effects. In general, probiotics are not a mainstay of clinical practice in North America. For example, an analysis by a high school student of physician practices in a small Canadian city showed that only 31% had any knowledge of probiotics and 24% felt that probiotics had no place in their practices 34 ; . The 31% figure may be much higher in many parts of the Western world, and of those who have knowledge, the accuracy of their information may also be flawed. For example, by definition, yogurt per se is not a probiotic, and many so-called acidophilus products have never been tested and do not fulfill the FAO and.

In my opinion, the arguments in favor of PABD have been largely exhausted, but there are still a few sincere advocates of PABD. They site unknown viruses that may yet turn up in the blood supply. To the best of my knowledge there are no unexplained blood borne epidemics at present. The West Nile Virus is a good example of the blood banking community's rapid response to a new pathogen. The molecular technology has advanced so far that we are unlikely to face the prospect of an unidentifiable virus like what we went through with HIV and HCV. Another argument cited for PABD is immunomodulation caused by white blood cells in the donor blood. Immunomodulation was first noted decades ago when renal transplant patients who had been transfused with blood had improved allograft survival. Whether this effect is significant today is subject to debate. Several studies comparing the outcomes of patients who had been transfused versus those who had not, showed risks such as increased post-operative infection rates or earlier recurrence of colon cancer in patients that had been transfused. Follow-up studies that took into account confounding factors like the duration of surgery, the size of the tumor, the preoperative hemoglobin, the prior health of the patient, etc. found that patients who required transfusions had worse outcomes because the same medical problems that necessitate transfusion contributed to their overall worse prognosis.16 Bronson Hospital in conjunction with the American Red Cross has adopted a policy of universal leukoreduction. Rapid ventricular rate during AF induced during electrophysiological testing. During follow-up, however, very few patients developed symptomatic arrhythmias, and none of these individuals experienced a cardiac arrest. The positive predictive value of invasive electrophysiological testing is considered to be too low to justify routine use in asymptomatic patients.83 The decision to ablate pathways in individuals with high-risk occupations, such as school bus drivers, pilots, and scuba divers, 83 is made on the basis of individual clinical considerations. These recommendations are likely to remain unchanged despite the results of a study that identified the results of electrophysiological testing as an important predictor of arrhythmic events in patients with asymptomatic pre-excitation.84 This study reported the follow-up of 212 patients with asymptomatic pre-excitation, all of whom underwent a baseline electrophysiological study. After 3816 months of follow-up, 33 patients became symptomatic, and 3 of these patients experienced VF resulting in death in 1 patient ; . The most important factor in predicting outcome was the inducibility of AVRT or AF during the baseline electrophysiological study. The presence of multiple accessory pathways was also identified as a predictor of future arrhythmic events. Of the 115 noninducible patients, only 3.4% developed a symptomatic supraventricular arrhythmia during follow-up. In contrast, 62% of the 47 inducible patients developed a symptomatic arrhythmia during follow-up including the 3 patients who experienced VF ; . Patients with asymptomatic pre-excitation should be encouraged to seek medical expertise whenever arrhythmia-related symptoms occur. The potential value of electrophysiological testing in identifying high-risk patients who may benefit from catheter ablation must be balanced against the approximately 2% risk of a major complication associated with catheter ablation. Summary of management In general, patients who have WPW syndrome i.e., preexcitation and symptoms ; , and particularly those with hemodynamic instability during their arrhythmia, should undergo catheter ablation as first-line therapy. Patients who experience infrequent minimally symptomatic episodes of SVT who do not have evidence of pre-excitation can be treated with a variety of approaches. These patients with concealed accessory pathways can be managed as patients with AVNRT. Patient preference is always an important consideration. Catheter ablation has sufficient efficacy and low risk to be used for symptomatic patients, either as initial therapy or for patients experiencing side effects or arrhythmia recurrence during drug therapy. Table 5lists recommendations for long-term therapy of accessory pathway-mediated arrhythmias, for example, tabamet hives. Flutamide has been used in patients with hirsutism, where 20 patients showed an increase in testosterone-binding globulin during therapy, with minimal side effects observed. Further trials are necessary to determine its efficacy for scalp hair growth in women with AGA. Cimetidine Tagam3t ; is a gastric H2-receptor antagonist also found to act as a weak antiandrogen by competing with DHT. Patients are given doses of 300 mg, five times a day, for 5 months or longer. Keep in mind that no controlled clinical trials have been done in men or women to show the efficacy of this drug in AGA. Dexamethasone or glpcocorticoids in general have been used to suppress adrenal and ovarian androgen production. Prescribed oral doses of 0.25 to 0.75 mg each night are given to reduce adrenocorticotropic hormone stimulation. Even though some patients have shown improvement of their hair loss or hirsutism, no large-scale clinical trials have been performed with this drug to show its effectiveness in alleviating androgen-dependent hair loss versus the multiple side effects that can occur with prolonged glucocorticoid use and tramadol.

Dr. Petracek: If you really want to do preventive medicine, this is one area where it could be improved.

After coronary bypass grafting is from 0.6% to 15%. There are different etiologies of these vascular events but the carotid lesion seems to be the principal etiology. There is no doubt that carotid endarterectomy for symptomatic carotid stenosis must be performed before coronary bypass surgery in patients with stable angina or simultaneously in patients with unstable angina. However there is a lack of evidence, in terms of controlled trials, in patients who need coronary revascularization and present asymptomatic carotid disease. The ACST recommends carotid endarterectomy in healthy men and women under 75 years. The 5-year reduction of neurological vascular events of this group is 6%. The author of this lecture presents one study whose aim is to determine the prevalence of carotid stenosis in coronary surgery patients and determine the timing of carotid and coronary surgery. They review 1250 consecutive carotid duplex studies in asymptomatic patients before coronary surgery. The overall incidence of severe carotid stenosis was 7.36% with this distribution: - Unilateral stenosis 70% ; 4.32% cases - Unilateral stenosis 70% ; and contralateral occlusion 0.64% - Bilateral stenosis 80% in both sides ; 2.4%. This incidence increases with age and is not different and vardenafil.
